from:
October 29, 2007
through
November 11, 2007
You MUST update the
following information:
1. COB forms, if your spouse/dependent
has other insurance coverage and you are receiving a
reimbursement for their insurance premium
2. If you are enrolled in a FSA account
and want to continue it for 2008.
If you are not affected
by the above two items then you don’t have to do anything.
Goodyear will automatically set you up next year the same
way you are set up this year.

Delta Dental Plan
As you know,
we recently negotiated the renewal of our Delta Dental
Plan. This plan is available for people who were hired
prior to October 1, 2006. The plan was effective as of July
1, 2007.
Although we
did still owe Delta Dental some money from the past
contract, we had a surplus of money with Goodyear so we were
not only able to keep all the benefits that we previously
had but were able to enhance the coverage a little bit,
while lowering the cost. The enhancements include dental
implants, coverage for composite resin fillings in the
posterior teeth and discounts if a Delta PPO Dentist is
used.
The dental
implants will be covered on a 50% basis. The composite
resin (tooth colored) fillings used to only be covered in
the front teeth. Before, if you had a composite resin
filling in a posterior (back) tooth, Delta would make an
allowance for an amalgam (silver) filling and you were
responsible for the difference. Under the new plan, these
composite resin fillings will now be covered the same as an
amalgam filling. Although our current Delta Premier network
of dentists DID NOT change, we did add language for a
“passive” PPO plan. That means if you should use a PPO
dentist, then we would get the discounts offered under that
network. For example, if you are going to a dentist that is
in both the Delta Premier Network and the Delta PPO Network,
the dentist would have to allow the PPO write-offs. Not
only would this save our plan money, it would also save you
money on the benefits that you have to pay on like crowns,
dentures and orthodontics. Since the dentist would charge
less, your portion would also be less. But remember, you do
not have to go to a PPO dentist if you do not want to. If
you do not go to a PPO dentist then your benefits would be
processed the same as they had been under the Delta Premier
network.
Along with
this explanation of the enhancements to our plan, I have
decided to also review the rest of the existing dental
plan. I do get calls from people asking about our dental
plan so, hopefully, this will help answer some of the
questions.
Benefits paid
at 100% of the usual, reasonable and customary charge
include:
· Exams
and cleanings – two in a twelve month period.
· Bite
wing x-rays to detect cavities – once in a twelve month
period.
· Complete
mouth x-rays – once every five years.
· Topically
applied fluorides – not for patients over age 19 except for
special need.
· Sealants
– once per tooth for first permanent molars up to age 9 and
second permanent molars up to age 14.
· Space
Retainers – when necessary for children who have prematurely
lost teeth.
· Fillings
– Amalgam and Composite Resin.
· Endodontics
– root canal when necessary.
· Periodontics
– gum disease.
· Oral
Surgery – surgical procedures, including extractions and
impacted teeth.
· Brush
Biopsy – oral cancer detection.
Benefits paid
at 80% of usual, reasonable, and customary charge:
· Major
Restorative – Crowns, caps, and other major restorative
procedures when fillings cannot restore the tooth. There is
a seven year time restriction on replacement.
Benefits paid
at 50% of usual, reasonable, and customary charge:
· Prosthodontics
– Placement of fixed bridgework as well as partial or full
removable dentures. There is a seven year time restriction
on replacement.
· Orthodontics
– prevention or correction (straightening) of malposed teeth
in dependent children under age 19. The maximum lifetime
benefit is limited to $1500.00.
· Dental
Implants – when necessary.
Our Delta
Dental Plan carries a maximum annual benefit amount of
$2,000.00 per dependent per benefit year. The benefit year
runs from July 1 to June 30 of each year. Excluded in this
annual maximum are procedures for Orthodontics, Periodontics,
Endodontics and Oral Surgery.
This is a
summary of your Delta Dental Plan. There are some
exclusions and some other limitations not mentioned in this
